I worked for a company who supplied Holdens at Elizabeth and I can’t say I am entirely surprised. I think a problem arises from the fact that they sacked so many seemingly surplus staff in the way of administration that a lot of the procedures that may have prevented this no longer exist. I won’t go into detail but I know of situations where certain things were ordered that would not have met procedural requirements.
