I like to hold a lit zippo lighter an inch from my gas tank while refueling w/ my cell phone in my other hand.

Never saw that episode, but you're probably right there.

However, they proved that shooting a bullet at the fuel tank does nothing.

Mythbusters did deign that mobiles won't blow your fuel up, they did however prove that static is a big worry. Doing it with the car running would only cause a problem if there's a bit of a power leak in your car which would build up a static charge/spark somehow near the fuel intake.

Cars being a "Faraday Cage" wont save you from making some mistakes that lead to static electricity, i remember seeing a video of a girl in a suv, pulled over for gas, switch the pump to auto, RETURNED to the car and came back out again to hold the gun, the second she aproach the gun with her hand the gas fumes ignited from a spark produced by static betwen her and the car, the car didn't blew up becouse it's safety valve shut down and the Air/gas mixture inside the tank wasn't proper. All gas pistol have a ground wire, thats why the triger is made out of metal, you have to discharge static when you pick up the gun, so even if you are trying to make a spark you can't, her mistake was to go back inside the car, recharge her body and aproaching the car (near the gas cap with fuel going in the car) leading to a spark caused by static, thats why "auto" gas pistol should be abandoned.

If you have two diferent groundings it's posible that the voltage diference betwen them will cause a spark like betwen a person and a car witch can light fuel gas if the mix is right. To lean and it wont ignite and too fat it wont ignite.

If the car is running don't realy mather since it stil has the same potential.
Toutch the car with your hand befour filling ant the votage diference will be lost.

Change driving instructor since they should know bether than telling people thing's that is not true.

It happens, many cars are stolen because the owner leaves the keys in the ignition while they go to pay.
